About Us

PharmaCare Institutional Services is the region’s premier and independently-owned pharmaceutical provider of complete pharmacy services to long-term care and assisted living facilities.

Our professional staff includes pharmacists and nationally certified pharmacy technicians, along with registered nurses and licensed respiratory therapists. All pharmacists are board certified and members of various state and national pharmaceutical organizations. Each member of our pharmacy team is here to answer questions and assist in making the appropriate decisions regarding each facility’s pharmaceutical needs. PharmaCare’s Institutional Services extends across a variety of long-term care facilities including:

Our institutional pharmacy operates 365 days a year with round-the-clock emergency service. An on-call pharmacist is available after hours to assist with all emergencies. Our pharmacy delivers twice a day (Mon - Fri) and once a day on Saturday and Sunday with emergency delivery service.

Services

Dispensing System

PharmaCare offers blister card packaging. For our blister cards we have partnered with Manchac using their DOSIS L60, a computer blister card packaging robot. This system is barcode driven and is interfaced with our pharmacy management system to ensure the safety and accuracy of each dose.

Pharmacy Consulting Services

Our consulting pharmacists are medication therapy experts responsible for completing comprehensive chart reviews at nursing facilities and assisted living homes. In addition, lead consulting pharmacist, Jackie James is a Board Certified Geriatric Pharmacist (BCGP) meaning she has achieved special certification in advanced pharmaceutical care of older patients.

Drug regimen reviews are completed on all new and readmissions, and appropriate recommendations are communicated to the resident’s nursing and prescribing team. The reviews include monitoring for safe and appropriate use of medication customized to each resident’s condition in accordance with CMS guidelines, and also include cost-saving alternatives.

Jackie consulting

Our consulting team attends quality assurance meetings and are integral members of the infectious disease and psychotropic review committees in our long-term care facilities. These medication and senior care experts are here to serve your community and long-term care patients/resident’s needs.


Quality Assurance & Quality Improvement Nursing

PharmaCare offers the services of an experienced long-term care nurse to visit your facility routinely, or on an as-needed basis for quality assurance and improvement. Our QA/QI nurse provides quality assurance/improvement audits by performing chart reviews, medication pass audits, medication cart audits, environmental audits plus monitors and evaluates that the facility is in compliance with all pharmacy policies and procedures. A detailed report is offered to each Director of Nursing and Administrator for their review. Our QA/QI nurse is also available as needed for specific projects at the request of the facility.


Infusion Therapy Nursing Services

PharmaCare Infusion Services offers a dedicated staff of registered infusion nurses to meet the infusion needs of long-term care residents, as well as, infusion training needs for their staff. Nursing facilities contracted with PharmaCare Infusion Services receive hands-on infusion therapy training along with products and services to care for patients with a variety of infusion therapy needs. PharmaCare PRN

PharmaCare PRN is a web-based application that is the facility’s gateway to everything our long-term care pharmacy has to offer. This portal allows the facility to securely connect to the pharmacy and obtain real-time resident, drug and facility-specific information. Modules include:

  • Message Center - secure/HIPAA compliant messaging to and from the pharmacy
  • Patient Profiles – complete patient profiles with individualized reporting
  • Drug Information – drug specific monographs, Medicare Part D formulary checker
  • Order Processing – place new orders and request refills, plus discontinue orders electronically
  • Price Quotes – generate price quotes and have pharmacist review for alternative(s)
  • QA/QI Reporting – built-in QA reports and monthly clinical report downloads
  • A/R Information – review past facility accounts receivable statements
  • Bookmarks – hyperlinks to pharmacy policy/procedures, Medicare Part D websites and prior authorization tools

Electronic Medical Records (EMR/EMAR)

PharmaCare uses QS/1’s PrimeCare pharmacy management system for their pharmacy dispensing. The PrimeCare system provides optional interfaces that improve efficiency, reduce duplication and improve customer service by interfacing with dozens of other products and technologies including electronic medical records (EMRs) by using government and industry standards such as HL7, XML, Web Services and NCPCP SCRIPT to ensure reliable and accurate communications from the facility to the pharmacy. PharmaCare currently supports interfaces with the following EMR providers:
